Our aides work under the supervision of a registered nurse. For each client, a registered nurse will conduct an initial assessment with the client and develop an individualized care plan. This care plan is designed to meet each client’s unique needs and is developed in accordance with the physician’s orders. Personal care services corresponding with the care plan are then assigned to an aide, and skilled nursing services are assigned to a registered nurse.
Our registered nurses provide skilled medical care under the direction of a physician to support health, recovery, chronic condition management, and overall well-being at home.

Skilled nursing services may include:
-
Comprehensive nursing assessments
-
Medication management and education
-
Disease and chronic condition monitoring
-
Wound care and dressing changes
-
Vital sign monitoring and health evaluations
-
Diabetes management and education
-
Catheter care and other physician-ordered treatments
-
Post-hospitalization support and recovery assistance
-
Patient and caregiver education to promote safety and independence
Our nurses collaborate with physicians and other healthcare professionals to help ensure continuity of care and support positive health outcomes.
